Whether you had a Loss-time Injury or RIDDOR, a pressure test failure or an Electrical Near Miss incident, I offer a complete suite of expertise that can undertake a comprehensive investigation for generic or technical Mechanical, Electrical or Plumbing contractors realted incidents.

The investigation process usually takes 2x weeks in which we compile all the findings, carry out necessary meetings and produce a final investigation report, together with outlining practical control measures & corrective action to prevent the reoccurrence.

Initial Incident Notification Report

Upon being notified of the incident, I will gather findings witnesses, conduct interviews, etc. This information will be compiled in an initial incident notification report that you can forward to your clients, notifying them of the incident whilst the full investigation is underway.

Following the completion of Step 1, an initial incident review meeting will be held with your project team, board of directors and (if required) your clients. Findings of the Initial incident notification report will be discussed, and further course of action will be agreed upon.

Full Investigation Report

Following the first two steps, the incident will then be analysed, assessed and perceived through various angles. Root Cause Analysis, Accountability Test and learning outcomes of the incident will be put together in an 8 - 10 pages Comprehensive Investigation Report.

Close-out meeting

A second meeting will be scheduled within 2x weeks of the incident occurring with the relevant parties to review the full incident investigation report's outcomes and agree on the corrective actions.

Bonus Step - Staff Breifing

As a bonus, all of my Incident Investigation packages will also come with an Incident briefing bulletin, created in simple wording that can be comprehended and understood by all personnel regardless of their level of English.
